Unique details about Texas Medical Board:

  • Texas Medical Jurisprudence Examination is required by all applicants

  • The Texas Medical Board requires a Personal Appearance of all applicants

  • The Texas Medical Board's fee for physician licensure is $805.00

  • The Texas Medical Board Does not offer a Temporary Medical License

  • The Texas Medical Board processes more medical license applications than most states.

Eligibility Requirements: (feel free to give us a call and we can tell you if you qualify for Texas Licensure)

Premedical Education
You must have completed either 60 hours of pre-medical education or you must have completed the required pre-medical education of the country where your medical school is located.

Medical Education
You must be a graduate of a U.S. or Canadian medical school or a graduate of an acceptable unapproved medical school. Fifth pathway applicants must have completed a Fifth Pathway program in lieu of graduation from medical school.

An acceptable unapproved medical school is defined as a school or college located outside the United States or Canada that:

(A) is substantially equivalent to a Texas medical school; and
(B) has not been disapproved by another state physician licensing agency unless the applicant can provide evidence that the disapproval was unfounded.

International Medical Graduates Additional Requirements
Must be:

  • ECFMG certified
  • able to communicate in English
  • eligible to practice in the country where your medical school is located
  • have supplied all additional information that the board may require concerning your medical school

You are advised to review the list of schools whose graduates do not have to prove substantial equivalence of their education. If your school is not on this list you will be required to submit several additional items for review.

Licensure Examinations
You must have passed an examination acceptable to the board. The most common examinations are USMLE and COMLEX. Refer to Board Rule for a complete listing.

All steps of your licensure examination must have been passed within 3 attempts and all steps must be passed within 7 years. There are some exceptions regarding the time limit - refer to Board Rule for more information.

Texas Medical Jurisprudence Examination
You must pass the Texas Medical Jurisprudence Examination with a score of 75 or better within three attempts (except for some Relicensure applicants). The examination is offered at Prometric testing sites throughout the United States. Once you have applied you will be sent a permission to test form. The form will contain instructions on how to enroll for the examination.

Postgraduate Training
You must have completed postgraduate training in the United States or Canada.

  • US/Canadian Graduates: one year of graduate medical training in the United States or Canada as defined in Board Rule
  • International Graduates: a three-year program of graduate medical training in the United States or Canada as defined in Board Rule

Professional Character
You must be of good professional character. An applicant for licensure must not be in violation of, or committed any act described in the Medical Practice Act, Tex. Occ. Code Ann. §§164.051-.053.

Personal Appearance
All applicants are required to appear at the boards offices for a personal appearance and inspection of original documents. You will be contacted by a Licensure staff member when your application is complete, and you may then schedule your appearance. Appointments are usually available every weekday except Wednesday. Appointments are not scheduled during holidays or board meetings.

Documentation required by the Texas Medical Board:

Alternate Name Documentation
Each applicant who has recorded an alternate name on the application must submit copies of documents that will support the name change. Those documents could be a marriage certificate, divorce decree, court ordered name change or some other official document.

Birth Certificate or Passport
copy of the applicants birth certificate or passport.

Clinical Clerkship Affidavit - Form J
If you are a US or Canadian medical school graduate, this document is not needed. If you are an international medical school graduate, complete and submit Form J.

Educational Commission for Foreign Medical Graduates (ECFMG) Certification
If you are a US or Canadian medical school graduate, this document is not needed.

Eligibility to Practice in Country of Graduation
If you are a US or Canadian medical school graduate, this document is not needed. If you are an international medical school graduate, submit a copy of your license to practice in the country where your medical school is located. If you do not have a license, you must have the licensing authority in the country where your medical school is located submit a letter to you regarding your eligibility to practice medicine. This letter must be in a sealed envelope with the licensing authorities seal stamped across the outside flap. (This requirement does not apply to Fifth Pathway applicants.)

Examination Transcripts
contact the appropriate examination agencies and request that a certified transcript of all examination attempts be sent directly to the Texas Medical Board.

Fifth Pathway Certificate
Fifth Pathway applicants must submit a copy of his/her Fifth Pathway certificate.

Fifth Pathway Letter
Fifth Pathway applicants must have the director of his/her Fifth Pathway program submit a letter outlining the methods used to evaluate and admit the applicant to the program. This letter must be in a sealed envelope with the program directors signature stamped across the outside flap. Forward the letter, in the sealed envelope, to the Texas Medical Board.

Medical School Diploma
If you are a US or Canadian medical school graduate, a copy of this document is not needed. If you are an international medical school graduate, submit a copy of your medical diploma, and a translation if the document is not in English.

Medical School Transcript (Certified)
you must request that each medical school you attended submit certified transcripts to you in an unopened envelope(s) with a signature of a medical school official across the outside envelope flap. Medical school transcripts and Form D may be submitted by the school in the same envelope.

Deans Certification - Form D
Form D and medical school transcripts may be submitted by the school in the same envelope.

National Practitioner Data Bank and Health Integrity Data Bank Reports
All applicants must contact the National Practitioner Data Bank/Healthcare Integrity and Protection Data Bank (NPDB/HIPDB) and perform a self-query. Submit both the final report from the NPDB and the final report from the HIPDB. These are two separate reports. Many applicants who have performed the self-query assume they have received two copies from NPDB/HIPDB as one report. However, close scrutiny will reveal that they are slightly different reports and the applicant received one report from each entity. Both must be submitted to this agency.

Personal Appearances
All applicants are required to appear at the boards offices for a personal appearance. During this appearance an inspection of documents will occur. You will be notified by a Licensure staff member when to appear. Routinely appearances are schedule for Monday, Tuesday, Thursday or Friday or most weeks. Staff does not schedule appearances during holiday or board meetings.

Prometric Name Verification Form - Form Z
All applicants must complete and submit Form Z, which is required to verify that the name submitted by Texas Medical Board (TMB) to Prometric for the Texas Medical Jurisprudence exam matches the name on your identification documents.

Reissuance Applicants
Applicants whose license has been revoked by the board are required to meet current statutory requirements and board rules. In addition physicians in this status must furnish documentation that the reissuance is in the physician's best interests and the best interest of the public. You are advised to submit any documentation that would have occurred since you were originally licensed or any documentation that is non-static, such as state board verifications.

Relicensure Applicants
Applicants whose license has been canceled for non-payment or by request are required to meet current statutory requirements and board rules. You are advised to submit any documentation that would have occurred since you were originally licensed or any documentation that is non-static, such as state board verifications.

Specialty Board Certification Documentation
Each applicant who is specialty board certified by a board that is a member of the American Board of Medical Specialties or the Bureau of Osteopathic Specialists must submit a copy of the most recent certificate awarded.

State Board Licensure Verifications
All applicants must contact each state, province or federal locate in which you have ever held a license or applied for a license and request that they furnish you a verification in a sealed envelope with the state board seal across the outside of the envelope. Some states may require that the verification be sent directly to the Texas Medical Board.

Texas Medical Jurisprudence Examination
Applicants for physician licensure (except for some Relicensure applicants) are required to pass the Texas Medical Jurisprudence examination (JP). The examination is offered at Prometric testing sites throughout the United States. Once you have applied you will be sent a permission to test form. Verification of Postgraduate Training/Physician Licensure Evaluation - Form L
you must have every US/Canadian training program with which you were ever affiliated complete Form L. In addition, for every professional affiliation you have had in the past 10 years, no matter the location, we require a completed Form L. Have the Chief of Staff, Program Director, Medical Director, Chairman of the Department or other supervising physician complete the form and return it to you in a sealed envelope. Do not open the envelope; send the unopened envelope to the Texas Medical Board.
